Neck Pain
Expert Physiotherapy in Bromley & Shortlands
Neck pain is one of the most common musculoskeletal complaints, affecting people of all ages and activity levels. It can range from mild stiffness after a night's poor sleep to severe pain that limits your ability to drive, work at a desk, or turn your head comfortably. In many cases it is accompanied by headaches, shoulder tension, or nerve symptoms that travel down the arm.
At Physio Squared in Shortlands, Bromley, neck pain is assessed and treated with a hands-on, clinically reasoned approach — looking not just at where it hurts, but at the full picture of how the cervical spine is moving, what structures are involved, and what is driving the problem.
What Causes Neck Pain?
The cervical spine is a highly mobile structure that supports the weight of the head while allowing rotation, flexion, and extension. This combination of mobility and load means it is vulnerable to both gradual wear and sudden injury.
Common causes of neck pain include:
Postural and occupational strain Prolonged desk work, sustained forward head posture, and repetitive movements at work are among the most frequent drivers of neck pain in adults. The muscles and joints of the upper cervical spine are particularly sensitive to sustained loading in poor positions.
Cervical joint dysfunction The facet joints of the cervical spine can become stiff, inflamed, or irritated — producing localised pain, restricted movement, and often referred pain into the head or shoulders. Joint dysfunction is common after sudden movements, sleeping awkwardly, or following periods of high stress.
Cervical radiculopathy When a nerve root in the cervical spine becomes irritated or compressed — often as a result of a disc bulge or osteophyte — pain, pins and needles, or weakness can radiate down the arm into the hand. This is sometimes called a trapped nerve and requires careful assessment to differentiate from shoulder or elbow problems.
Whiplash and trauma Whiplash-associated disorder following a road traffic accident or sudden impact can affect the muscles, ligaments, joints, and discs of the cervical spine simultaneously. Early, guided physiotherapy is strongly associated with better outcomes in whiplash recovery.
Cervicogenic headache Headaches that originate from the joints and structures of the upper cervical spine are classified as cervicogenic. They are frequently misdiagnosed as tension headache or migraine. A skilled physiotherapy assessment can identify the cervical origin and treat it directly — often with rapid relief.
Muscle tension and stress Psychological stress manifests physically in the muscles of the neck and shoulders. Persistent muscle tension creates a cycle of pain, guarding, and restricted movement that can become self-perpetuating without treatment.
Assessment at Physio Squared
A thorough assessment is the foundation of effective treatment. At Physio Squared, this includes a detailed history of how and when your neck pain developed, a postural and movement screen of the cervical and thoracic spine, neurological testing where arm symptoms are present, joint palpation to identify specific levels of dysfunction, and where relevant, a biomechanical assessment of how the whole spine and shoulder girdle is contributing.
This level of assessment takes time — and as an independent clinic, that time is available. The aim is not just to treat the symptoms but to understand why they developed, so treatment actually addresses the cause.
How Neck Pain is Treated
Treatment is tailored to the assessment findings and your goals. The approaches used at Physio Squared include:
Manual therapy Hands-on mobilisation and manipulation of the cervical and thoracic spine to restore range of movement, reduce joint stiffness, and relieve pain. Manual therapy is one of the most evidence-supported interventions for mechanical neck pain and cervicogenic headache.
Medical acupuncture Western medical acupuncture can be highly effective for neck pain, particularly where muscle guarding, trigger points, or persistent pain are features. It is used alongside manual therapy rather than as a standalone treatment.
Exercise prescription Targeted rehabilitation exercises to restore deep cervical flexor strength, improve postural control, and reduce the likelihood of recurrence. These are explained clearly and progressed at a pace appropriate to your recovery.
Ergonomic and postural advice Practical, specific guidance on workstation setup, sleep position, and daily habits that are contributing to your symptoms — not generic advice, but recommendations based on your assessment findings.
Load modification For active patients or those returning to sport, a structured plan for managing neck load during the recovery period — allowing you to stay active safely while the cervical spine recovers.
When to Seek Urgent Assessment
Most neck pain is mechanical and responds well to physiotherapy. However, some symptoms require urgent medical attention. Seek immediate care if you experience severe neck pain following trauma, significant arm weakness, loss of bladder or bowel control, dizziness or difficulty walking alongside neck pain, or pain that is constant, progressive, and not affected by position.
If you are unsure whether your symptoms are appropriate for physiotherapy, call or email to discuss — a brief clinical conversation can usually provide clarity.
